2024 Options Counseling Job Aid and Video Instruction

The 2024 Options Counseling Job Aid provides information on the OC data entry process in the Aging & Disability (A&D) database. Although much of the Job Aid pertains to A&D users, the Appendix contains information that is useful to both ASAP and ILC staff who are responsible for entering OC data into their system.

To accompany the Job Aid, a 15-minute video, narrated by Brendan Carroll, provides instruction on the OC Data entry process in A&D. It is a helpful companion to the 2024 OC A&D Job Aid and is most useful to ASAP staff, as it provides specific instruction on how to enter the OC data in the Aging and Disability database. Family Caregiver Support Program and Options Counseling Service Reporting Protocol

Attached is the Family Caregiver Support Program and Options Counseling Service Reporting Protocol. This document describes the process that should be used in very specific instances by staff members whose roles include that of both Family Caregiver Support Specialist and Options Counselor. 

The process  captures those instances when during the course of an Options Counseling session, that includes both a consumer (elder or person with a disability) and a caregiver, 15 minutes or more is dedicated to providing family caregiver support, solely. Please refer to the attachment for details.

Options Counseling Program

 When an older person, or an adult of any age with a disability, faces the need for supportive services to help with basic activities such as personal care, household chores, transportation, nutrition or medication management, locating and evaluating options for care can be an overwhelming task for the individual or their family member or caretaker. The process can be made more manageable with the support of a knowledgeable, caring counselor who provides unbiased information about the range of community and institutional services and how to access them, and who supports the individual during the decision-making process by ensuring that the individual understands the range of available options, in order to make an informed choice, and to determine next steps to take. The service is provided in person, by phone, and via email or the web, and may be accomplished in a single session or over several sessions

 Options Counseling services are state-wide and intended to be short-term in nature and nay include phone calls and/or face-to-face meetings. The timing and the number of sessions provided depend on the consumer’s individual need for information and decision-support. Consumers may receive services in a hospital, rehabilitation or nursing facility or their own homes.
